Children MUST be up to date with required immunizations prior to starting childcare. This link provides information as to what and how and many immunizations your child should have at what age. Copy of immunization record must be presented at time of enrollment.

If you choose to enroll your child with Colleen’s Child; these forms MUST be printed and filled out prior to the child’s first day of care.
